    I have seen the increase of patients visiting Endodontic specialized clinics like ours searching for a hope to hear the better treatment options as a second opinion to their root canal treatments which once diagnosed as a hopeless.
    In my first part of presentation, I wish to share with you the Endodontic case which had been diagnosed as a hopeless tooth under the conventional theory of Endodontic treatment, performing removal of infected dentin under a magnified view, avoiding those common secondary disaster such as root perforations and/or root cracks caused from the root canal re-treatment. And at the same time, minimize the number of remaining bacteria in a canal results to maintain the low bacteria rated to avoid extraction.
    In my next part, I wish to introduce you and explain in details of some specialized Endodontic instruments which make the treatment possible, with their respective features and methods throughout my clinical cases.
    I wish that my lecture of today would become a one of the guiding light to encourage those colleagues who wish to take the higher challenges for the wistful demands of their patients desperate to keep their hopeless teeth of infected root canals which generally diagnosed as an extraction as the only option.

  • EXPLORING THE UNEXPLORED- Bilateralism
    Successful endodontic therapy requires a thorough knowledge of root canal morphology. There is a wide range of variations in the literature with respect to frequency of occurrence of the number of roots, the number of canals in each root, etc. But less attention has been paid to the bilateral occurrence. Generally rarer the aberration, more common is the bilateralism.
    Along with the regular and mesial/distal inclined preoperative radiographs, recommendations have been made to include a radiograph of the contralateral tooth. Incorporation of CBCT for diagnosis of the aberrations has made evaluation and treatment of these bilateral occurrences with more predictable results. A previously treated tooth with unusual root morphology may serve as an excellent reference to determine whether that aberration exists in the corresponding contralateral tooth that is being considered for therapy.
    This presentation explains about the importance of the clinician to think about the possibility of bilateral occurrence when he/she comes across any aberration in the root canal morphology and the importance of using modern gadgets like CBCT routinely for better understanding of the complexities of the root canal system and ensure a predictable treatment prognosis for such aberrations.

    With the incorporation of new techniques, endodontics has become more predictable in the 21st century than ever before. This lecture reviews the latest updates in endodontics and their incorporation in private practice. A modern practicable concept for the treatment of apical periodontitis in difficult cases is presented including adequate bio-mechanical instrumentation and disinfection. For cases with unsuccessful primary root canal treatment it will be discussed when to choose surgical endodontics over non-surgical retreatment, focusing on the predictability as well as the management of compromised cases, including perio-endodontic lesions and situations with anatomical and technical limitations.

    It is a sad fact that the quality of endodontic treatment worldwide is poor and with this the incidence of endodontic disease is high. This is excellent ammunition for those dentists who see implants as better alternative to endodontics.
    A poorly root treated tooth can be extracted or retreated. Understanding the cause of failure in primary endodontic treatment will help the general practitoner decide what cases can be retreated.
    The huge advances in endodontic technology such as the microscope and ultrasonics have simplified endodontic retreatment procedures to a level of consistent predictability where success in terms in elimination of disease and retention of the tooth in function can be a realistic outcome.
    The lecture will review the causes of success and failure and the scope of endodontic retreatment. The multimedia presentation will look at post removal, GP, carrier based obturation, separated instrument removal as well as perforation repair with MTA and Biodentine.
    Our patients want predictable outcomes and we can achieve this for them with tooth retention as the final goal

    Many studies show that nickel-titanium alloy is greatly superior to stainless steel, since with NiTi instruments, even canals with accentuated curvatures can safely be shaped without the risk of creating ledges or straightening the original curves. On the other hand, most NiTi breakages occur in the last few millimeters, where the instrument's taper is less and the diameter smaller. Reduction of breakage can be achieved when NiTi use is preceded by preliminary enlargement and the creation of a glide path, which is usually done by hand with stainless steel instruments. These instruments involve numerous disadvantages, due to their rigidity and aggressive tip. A new rotary file was designed to create the glide path quickly and safely, thus eliminating the last manual phase in which the general practitioner can commit errors, and providing the expert endodontist with a tool that can transform difficult cases into simple ones.

  • Esthetic Restorative Challenges with Composite
    Materials used for the esthetic restorative treatment vary from composite
    resin to ceramic. Highly frequent used composite resin as the least
    restorative treatment was acknowledged as ?one of the options of esthetic
    restorative treatment', however, it recently started to be admitted as ?simple
    and easy treatment for small cavities'. Furthermore, this treatment is an
    excellent treatment that would be applied for broader application range
    including region with occlusion forces. However, to apply this composite resin
    restorative treatment as esthetic restorative treatment, there needs to have a
    correct knowledge about adhesive bonding by clarifying the criterion of tooth
    preparation and also needs to have some trainings to master materials and
    equipments. Also these are technique sensitive procedure. By making ensure
    steps using microscope, we can raise the quality.

    In this presentation, I'd like to discuss about how to apply ?adhesive
    dentistry' with the use of the microscope to the actual esthetic restorative
    treatments by showing you clinical cases.

    In my lecture I want to show, how the use of a Microscope changes the way teeth are treated today.
    High resolution is widely accepted as the key to success in many fields, e.g. hubble telescope in space, surveillance in military actions, Neurosurgeons etc.. Not yet in general dentistry.
    Dentists who use a Microscope are mostly endodontists, periodontists or oral surgeons. The use of magnification combined with light should enable the general dentist to start an excellent work at the beginning of the "dental treatment-career", and not at the end of a "tooth-life". This is more efficient, more esthetic, cheaper and solves a lot of problems we still have, if we try to restore a patient with non removable prosthetics as a "young ager".

In order to achieve these, microscopically enhanced therapies provide a new way for insight into the solutions. Under the microscope,"flapless approach" is no more equated with"blind treatment". The utilization of this perfect visual tool offers an aid as an eye opener for the dental clinicians.     When maximum precision is required in performing surgery to satisfy particular esthetic demands, a surgical microscope may represent a useful tool. Normally we will use the Probing Pocket Depth or attachment gain as as success criteria. With a special procedure well known from ophthalmology called Fluorescence Angiography we are able to show the healing results direct past surgery and within the first days post OP. So we could optimise the material and handling of the tissue by visualising the blood flow depending on different techniques, suture materials and regenerative materials and procedures.
    The application of magnification in periodontal surgery accomplished better results in terms of success and predictability compared to conventional techniques.

  • The Use of Microscope-Level Magnification (6-8x or greater) as aids in performing Tooth Extractions in Dentistry
    The use of Microscope-level magnification (6-8x or greater) combined with head-mounted, co-axial illumination provides a tremendous improvement in a dentist's ability to extract teeth compared to if the dentist used unaided vision and non-coaxial operatory lighting. This presentation describes precisely in what ways high magnification and co-axial lighting provides advantages when extracting teeth. Magnification provides an improved ability to know where to section teeth and remove intra-socket bone, and greatly improves a dentist's ability to determine if elevators that are positioned around tooth particles are contributing to incremental improvements in the luxation of tooth particles. Microscopic differences in how elevators are placed around tooth particle perimeters affect the extent to which a certain way of placing an elevator around a tooth particle is resulting in microscopic incremental improvements in tooth particle luxation. A key concept is that the accumulation of microscopic incremental improvements in tooth particle luxation results in MACROscopic movements of tooth particles. And the key to sensing if microscopic incremental improvements in luxation are occurring is, simply, for a dentist to be able to view the extraction site using microscope-level magnification and head-mounted lighting. The unaided vision dentist cannot sense these microscopic incremental improvements in the magnitude and direction of tooth particle luxation, resulting less efficient use of elevators.
