Course Evaluation Survey

You must be logged in to fill out the course evaluation surveys for your CE credits.

Live CE Event:

Most state boards define a live seminar as a seminar that is hosted in a live (not prerecorded) format and allows interaction by the attendees with the seminar presenter. By this definition, the "live" webcasts presented by AMED would count as live seminar continuing education units. Please check with your state's licensing board for exact requirements.

All live participants will be given a certificate of attendance per your request.

Task List:
1. Complete each presentation evaluation form.
2. At the bottom of the evaluation form page, there is a button to submit all the forms when you are ready to get your certificate.
3. We'll process you're forms and email you a certificate.

Self Instructional CE Credits:

The AMED meeting is available as coursework provided in a self-instruction format which includes a testing mechanism supplied by the presenter. In order to complete self instructional CE credits an evaluation form must be completed for each lecture. Links are located below each presentation, but they all link to the same page.

Dental Continuing Education Credits

AMED is an ADA CERP recognized provider of dental continuing education.
AMED is an AGD approved provider of dental continuing education.

ADA CERP is a service of the American Dental Association to assist dental professionals in identifying quality providers of continuing dental education. ADA CERP does not approve or endorse individual courses or instructors, nor does it imply acceptance of credit hours by boards of dentistry.

Concerns or complaints about a CE provider may be directed to the provider or to ADA CERP at www.ada.org/goto/cerp.

Continuing education credits issued for participation in the CE activity may not apply towards license renewal in all licensing jurisdictions. It is the responsibility of each participant to verify the CE requirements of his/her licensing or regulatory agency.
